Job Summary: As a collaborative provider on the primary care treatment team, this role identifies, triages and manages patients within the primary care setting. Provides excellent external and internal customer service resulting in quality patient care, high satisfaction and provider efficiency. Performs independent assessments, provides patient education and demonstrates initiative and creative problem solving to address patient concerns.
Job Requirements:
Nurse Practitioner: Masters Degree in Nursing required. Advanced Practice Nurse Prescriber (APNP) and Registered Nurse License in State of Wisconsin required. Certification as Nurse Practitioner by American Nursing Association.
Physician Assistant: Completion of a Physician Assistant Clinical Program as approved by the American Medical Association and licensure as a Physician Assistant in the State of Wisconsin. Certification by the National Commission on Certification of Physician Assistants (NCCPA). Must demonstrate a commitment to shared team goals and previous experience working effectively within a team structure. Previous experience working as a Nurse Practitioner or Physician Assistant within a collaborative primary care environment desired. This role requires the ability to embody an attitude of service, a willingness to maintain and increase competencies, positive attitude, creative solution based problem solving, and excellent listening and communication skills.
