Job Summary: TEMPORARY OPPORTUNITY -- Provides comprehensive assessments, care coordination, and care planning to Medicaid members at home, in skilled nursing facilities and/or assisted living CBRF/group homes as necessary in collaboration with Primary Care Provider - improving coordination, continuity, assessibility and appropriate utilization of health care services. Job Requirements: Masters prepared in Nursing. Licensed as a registered nurse in State of Wisconsin. Certification as Nurse Practitioner by American Nursing Association. Previous work experience preferred as a Nurse Practitioner 2-4 years, adult or geriatric experience preferred. Valid Wisconsin drivers license.
