BS or equivalent in Exercise Science, Nursing, or Health related field required. Masters degree preferred. Must have extensive knowledge of population/community health programs, outpatient to inpatient bridge programs, diagnostic health screening and diagnosis as well as health risk assessment programs.  5 year recent administrative experience in disease management and community health.   Prefer outpatient diagnostic programming experience. Must have positive, enthusiastic attitude and desire to work within a comprehensive health care team. Shows commitment to healthy lifestyle as a model for participants and staff.  Current SC licensure if applicable.  Connect Care e-ssentials module proficiency is required.
