This position is part-time in the CHN Walk-In clinic located at Berlin Memorial Hospital in Berlin, WI.  The hours are 11am - 7pm during week and 10am - 6pm on weekends.  Some weekend work required.

Summary:

The Nurse Practitioner (NP) or Physicians Assistant (PA) will be available during office hours and scheduled for the triage, treatment, and/or referral of patients of Community Health Network.  Assessment, diagnosis, treatment and/or referral will be made in accordance with the medical protocols on file in the clinic and the appropriate Wisconsin statutes and federal regulations.

Qualifications:

• Nurse Practitioner/Physicians Assistant licensure
• Experience in providing primary care service in a rural setting preferred.
• CPR certification is required.
