VCU Health System's Medicine Administration is seeking a full time ASAP Nurse Pt Advocate. This person provides university and health system leaders with a single point of contact for facilitating inpatient and outpatient visits to VCUHS for this patient population. Provides the targeted patients with a single point of contact to VCUHS for telephone triage, appointments, referral information, discharge care and home assistance and all necessary arrangements to ensure positive experiences.
Coordinates activities with patient and visitor relations, physicians` offices and clinic staff.
Serves as the spokesperson and recognized leader of the VCUHS SCP.
Develops evaluation process to measure the activity and impact of the program.
Must be flexible with work schedules according to department staffing need or shift requirements. May be required to work some weekends and holidays.
Baccalaureate Degree in Nursing from an accredited program
Current RN licensure in Virginia
AHA BLS HCP Certification or equivalent
Minimum of three (3) years of clinical nursing experience
Masters Degree in Nursing from an accredited program
Five (5) years of experience working at VCUHS with extensive knowledge of the organization, clinical leadership and scope of services provided
Extensive knowledge of available community healthcare resources to address constituents' needs
