VCU Health System's Rehab Med-Amin Department is seeking a part time RN Rehab Coordinator to facilitate the efficient, timely coordination of health care services provided to a defined patient population. Responsibilities include the daily monitoring of the patient medical readiness and appropriate tolerance and participation of therapies for evaluation and potential transfer for rehabilitation services. Identifies opportunities for improvement and coordinates and/or initiates interventions when opportunities are identified, facilitates improvement in organization performance, assumes a leadership/education role in the interdisciplinary health care team, and advocates for the patient/family at the service-delivery level. Serves as a rehabilitation consultant by making patient rounds at VCUHS and other facilities to market the VCUHS rehab program and responds to the need for education and consultation for potential rehab candidates.
Interprets criteria for specific case application, concurrent intervention when opportunities arise to educate the patient, family and /or significant other, referral sources, hospital personnel, external payors, and external case managers about the rehabilitation program benefits and services.
Meets with the patient and family to ensure understanding about rehabilitation admission process, readiness for rehab, and rights and responsibilities of patient/family.
Based on patient and family needs provides appropriate information for informed decision making. Answers questions and follows-up with patient and family on a timely basis.
Makes daily varied surgical team rounds at VCUHS and serves as a rehabilitation consultant to assist with the case management and transition of potential rehab patients, and offer options to the management of complex patients
Markets the VCUHS rehab program to other Healthcare facilities.
Re-consults patients on a daily basis and collaborates with health care teams to determine if the patient is appropriate for inpatient physical rehabilitation, if initially they were not deemed appropriate on the day of initial consult.
Provides consistent documentation, in the patient electronic record as to the progression of the patient regarding the rehabilitation consult process and potential barriers and or strategies needed to be utilized to meet readiness for inpatient physical rehabilitation.
Provides constant communication to the inpatient rehabilitation acute care team regarding the potential need of services related to the education of staff and management of the increase in medically complex potential patients.
Provides a continued presence, availability, awareness and contact of the consulted patient population until discharge as the PM&R consult service rotates.
Manages consult pager and communicates with the medical teams independently and communicate with the attending when the resident is not available and or in training sessions.
Based on patient and family needs provides appropriate information for informed decision making. Answers questions and follows-up with patient and family on a timely basis.
Works with the consult team members to provide services based on the arrival of consult requests and timeliness of referral evaluation.
Bachelor's Degree in Nursing from an accredited School of Nursing
Current RN licensure in Virginia
AHA, BLS, HCP Certification or equivalent
Minimum of three (3) of three years of acute care and/or long term care work experience with emphasis on rehab patients

Master's Degree in Nursing or a healthcare related field from an accredited program
Rehabilitation Registered Nurse Certification (CRRN)
