Job Details:

VCU Health System's Va Coordinated Care is seeking a full time Nurse Case Manager. This person demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort.



Makes appropriate decisions and initiates appropriate interventions in the provisions of care coordination within the boundaries of professional practice, the Standards of Practice for Case Management (CMSA) and established policies.

Organizes and plans work with input from the client/family with specific outcomes and demonstrated use of sound clinical judgment and resources.

Determines the appropriate place of contact/intervention with customer including client's home, community agency, provider office, etc. in order to achieve customer satisfaction and a positive outcome.





Baccalaureate Degree in Nursing from an accredited School of Nursing

Current RN licensure in Virginia

AHA BLS HCP Certification or equivalent

Three (3) years of experience including two (2) in a specialty area



Certification in adult general/internal medicine or specialty area as appropriate
Two (2) years case management/care management experience including two (2) in adult general/internal medicine in specialty area.
