VCU Health System's Social Work is seeking a full time ED Clinical Social Worker 1 to deliver discharge planning and social work services during normal business hours and the entire hospital after hours. Requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. Provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. Responsible for the daily management of all complex discharge planning and social work functions.
Primary responsibilities include proactive development, implementation and ongoing revision of an interdisciplinary plan of care for patients who receive care in the Emergency Department.
Negotiates and collaborates with the interdisciplinary team, community resources and payers to facilitate timely and cost effective discharge plans that enhance appropriate health and social services in the continuum of care.
Masters of Social Work Degree from a Council on Social Work Education accredited program
New graduate to up to 2 years of clinical social work experience*
*Advancement from a Clinician 1 to a Clinician 2 is mandatory within 2 years of employment into position
Licensed Clinical Social Worker in the State of Virginia
Clinical social work experience in an acute care setting
Related experience in acute discharge planning, home health, case management, long term care or related community agency
