The VCU Health NOW Clinic is seeking a full time Nurse Patient Navigator (TBI) that f
unctions as a Nurse Patient Navigator and serves as an advocate, interpreter, educator and counselor for a group of patients. Manages the care of assigned patients in collaboration with a multidisciplinary team to ensure high quality and comprehensive healthcare and ancillary services. Serves as the single point of contact to assigned patients and caregivers. Remains a support system to patients and caregivers throughout the patient's treatment. Uses clinical/nursing knowledge and skills in the performance of job responsibilities.
Specific Duties Related to This Job: Shared responsibility for the multidisciplinary triage and management of the patients in the neuroscience continuum of concussion and outpatient traumatic brain injury (TBI). Ages 5 years through 25 years. Coordinate care between 3 VCU clinic locations for concussion; 4 VCU clinic locations for all TBI. This navigator will be the direct contact for referring physicians, patients, coaches, recreational sports clubs, schools and caregivers to provide resources and assistance with accessing clinical information, scheduling appointments, and coordinating medical care. They will provide clinical support in the Pediatric and NOW facilities with Patient Access Representative (PAR) and Athletic Trainer (ATC) assistance.
Licensure, Certification, or Registration Requirements for Hire
Current RN licensure in Virginia or eligible or compact state
Licensure, Certification, or Registration Requirements for continued employment including CPR, etc.
Current RN licensure in Virginia
AHA BLS Certification
Specialty Nursing Certification in either Nurse Navigation
Level and type of experience REQUIRED
Minimum of three (3) years of relevant clinical experience demonstrating
Patient education, nursing, planning, case management or similar experience
Previous experience in specialty area of clinical concussion care and education
Level and type of experience PREFERRED
Demonstrated experience and excellence at multidisciplinary collaboration with clinical and community partners
Nurse Navigation or leadership experience
Experience working in a NCIC comprehensive Program
Experience within an academic, teaching hospital
Baccalaureate Degree in Nursing from an accredited School of Nursing
Master' Degree in Nursing from an accredited School of Nursing
