The VCU Health is seeking a full time Nurse Patient Navigator (TBI) that functions as a Nurse Patient Navigator and serves as an advocate, interpreter, educator and counselor for a group of patients.  Manages the care of assigned patients in collaboration with a multidisciplinary team to ensure high quality and comprehensive healthcare and ancillary services. Serves as the single point of contact to assigned patients and caregivers. Remains a support system to patients and caregivers throughout the patient's treatment. Uses clinical/nursing knowledge and skills in the performance of job responsibilities.

 

Functions as a Nurse Program Coordinator in the establishment of a new Pediatric Brain Injury Care program, to include: ongoing program innovation and development, patient navigation, participation in treatment protocol development and implementation, and quality improvement and research database development and maintenance. This position is specific to a 3-year Children's Hospital Foundation grant to improve pediatric traumatic brain injury (TBI) access, care, research, and education.  Serves as an advocate, medical interpreter, educator and counselor for a group of pediatric patients (0-18 years) and their families during and after hospitalization for acute brain injury, and facilitates transition into, and continuity within, the long-term recovery and follow-up process, to include collaboration with community primary care providers. Manages the care of assigned patients in collaboration with a multidisciplinary team to ensure high quality and comprehensive healthcare and ancillary services. Serves as the primary point of contact to assigned patients and caregivers. Remains a support system to patients and caregivers throughout the patient's treatment and in outpatient recovery. Uses clinical/nursing knowledge and skills in the performance of job responsibilities.

 

Licensure, Certification, or Registration Requirements for Hire

 

Current RN licensure in Virginia or eligible or compact state

 

Licensure, Certification, or Registration Requirements for continued employment including CPR, etc.

 

Current RN licensure in Virginia

 

AHA BLS Certification

 

Specialty Nursing Certification in either Nurse Navigation or area of specialty within one year of hire in position CNRN (Certified Neuroscience Registered Nurse), CBIS (Certified Brain Injury Specialist), CPN (Certified Pediatric Nurse), CCRN (Critical Care Registered Nurse

 

Level and type of experience REQUIRED

 

Minimum of three (3) years of relevant clinical experience demonstrating

 

Patient education, nursing, planning, case management or similar experience

 

Previous experience in specialty area of clinical concussion care and education

 

Level and type of experience PREFERRED

 

Demonstrated experience and excellence at multidisciplinary collaboration with clinical and community partners

 

Nurse Navigation or leadership experience

 

Experience working in a NCIC comprehensive Program

 

Experience within an academic, teaching hospital

 

 

Education/training REQUIRED

 

Baccalaureate Degree in Nursing from an accredited School of Nursing

 

Education/training PREFERRED

 

Master' Degree in Nursing from an accredited School of Nursing
