VCU Health System-Children's Hospital of Richmond (CHoR), Brook Road Campus, is seeking a full time Minimum Data Set (MDS) Nurse Coordinator to collect comprehensive data pertinent to the pediatric population or situation and analyze the assessment data to determine the diagnosis or issue(s).  The MDS Nurse Coordinator develops a plan of care to attain expected outcomes and directs the process.

 

Responsibilities

 

Collects data pertaining to residents' health or the situation and analyzes the data to determine the diagnoses or issues. Develops the plan of care to attain expected outcomes. Synthesizes available data, information, and knowledge relevant to the situation to identify patterns and variances.

Directs processes and communicates information to the interdisciplinary team and is a member of the interdisciplinary team.  Develops expected outcomes and individualized plans. Implements the plan and coordinates care. Evaluates resident's progress. Coordinates MDSC nursing activities. Must understand and interpret regulations pertaining to the MDS. May need to in-service staff on requirements and changes as they occur. Performs other duties as assigned and/or participates in special projects in order to support the mission of VCUHS and the Department.

Qualifications

 

Required

 

Baccalaureate Degree in Nursing from an accredited School of Nursing or Associate's degree in Nursing but must enroll in an accredited Baccalaureate Degree in Nursing program within 24 months of the data of hire and complete the degree no later than 5 years/60 months from the date of hire.

Current RN licensure in Virginia or eligible or compact state

CPR Certification

MDS Certification within one year of hire

Minimum of three (3) years of clinical experience

Charge Nurse or supervisory experience

 

Preferred

Long Term Care and MDS experience
