RN – Discharge Follow-Up Position Overview

The Discharge Follow-Up RN is responsible for making telephone calls to patients following discharge.  This is a key role in Population Health. Population Health is defined as the health outcomes of a group of individuals, including the distribution of such outcomes within the group. The Discharge Follow Up RN will call patients post discharge to assist with the transition from inpatient hospital admission to outpatient/ambulatory care. Telephonic calls to patients can vary from a single contact to weekly contacts for 30 days for patient needing chronic care management/high risk management.


   The Discharge Follow-Up RN will have the opportunity to:

·         Work closely with providers, nurses, and other members of the organization for seamless transition from hospitalization through post discharge care

·         Utilize critical thinking skills to assist patients with their needs post discharge

·         Work in a team oriented department


Hours will vary weekly. 8 hour days, 3 day's one week, 2 days the following week for a total of 40 hours per pay period. Program hours are Monday-Friday, no weekends at this time. Schedule can change based on program needs.




·         Associate Degree in Nursing required

·         Bachelor of Science in Nursing (BSN), preferred


·         Diverse clinical experience with at least 5 years of nursing experience in an acute care setting

·         Computer experience (with Excel and electronic health record system)

·         Extensive clinical knowledge of the medical/surgical population

Other required skills:

·         Ability to answer questions of patients and families and to make critical decisions

·         The ability to prioritize job responsibilities and organize workload

·         Critical thinking and problem solving skills to facilitate timely and appropriate decision making

·         The ability to function in a team environment with healthcare professionals

·         Excellent interpersonal skills to effectively represent department to the public and interact with fellow team members, nursing/physician/ancillary staff, third-party payors and patients/families.

·         Diverse clinical experience with 10 years nursing in an acute care setting, preferred

·         Experience in medical, surgical, maternal and child units, preferred

·         Project management skills, preferred

·         Leadership attributes, preferred

·         Ability to gather data and report on findings, preferred

·         Bilingual/Spanish speaking, preferred



·         Registered Nurse (RN) licensed by Oregon State Board of Nursing.  Will consider candidates with RN licensure from out of state – but Oregon license is required upon start 

·         Basic Life Support for the Healthcare Provider (BLS) certified through the American Heart Association (AHA), preferred
