Discharge Planner/Utilization Review Case Manager Overview

As a Discharge  Planner/Utilization Review Case Manager you will monitor utilization practices from preadmission to discharge to assure cost-effective utilization practices, quality patient care, and compliance with regulatory requirements.  Coordinates plans for discharges involving community resources and alternate levels of care to ensure timely and efficient case coordination in conjunction with members of the health care team. 


As a Discharge Planner/Utilization Review Case Manager at Asante, you will have the opportunity to:

·        Collaboration with many disciplines in the hospital and the community.

·        Promote transitions of care across the continuum.

·        Work with community resources to ensure the patient has a safe plan when they transition out of the hospital.


This position will be for discharge RN on General Medicine Floor. 



·         Associates Degree in Nursing or above

·         Bachelor's degree in nursing or allied health field, preferred


·        2 recent years experience in an acute care setting

·        2-4 years of demonstrated utilization/case management experience in a recognized program, preferred.

·        Ability to prioritize job responsibilities and organize workload required.

·        Ability to function in a team environment with healthcare professionals, required.

·        Must possess excellent interpersonal skills to effectively represent department to the public and interact with fellow team members, nursing/physician/ancillary staff, third-party payors and patients/families, required.

·        Must possess proficiency in interpersonal skills to effectively interact with fellow team members, nursing/physician/ancillary staff, third-party payors and patients/families.

·         Project management skills, preferred

·         Technical support with computer skills, preferred

·         Critical thinking, leadership attributes, and problem solving skills, preferred.

·         Ability to gather data and report on findings, preferred.


·        Registered Nurse (RN) licensed by Oregon State Board of Nursing.  Will consider candidates with RN licensure from out of state – but Oregon license is required by start date

·        Basic Life Support (BLS) certified by American Heart Association (AHA), preferred

·        Certified Case Manager (CCM) by Commission for Case Manager Certification, preferred

·        Certified Professional Utilization Review (CPUR) or Healthcare Management (CPHM) by McKesson, preferred
