Job Details:
  • Integrates Quality Assessment Performance Improvement (QAPI) activities with daily operations in assigned region(s)

     

    JOB RESPONSIBILITIES:

    ·         Performs focused reviews based on e.g. external survey findings, complaints, internal audit findings, new policy and procedure implementation, HPMS/DOH data

     

     

     

     

     

    ·         Conducts quality audits of membership development documentation to ensure accuracy, completion and compliance

     

     

     

     

     

    ·         Utilizes data and collaborates with Assistant Director to set priorities and develop quality improvement work plan

     

     

     

     

     

    ·         Utilizes various techniques, e.g. root cause analysis, to investigate potentially avoidable events

     

     

     

     

     

    ·         Facilitates quality focus groups to develop strategies to assist staff in developing interventions to improve outcomes; serves as a resource for QI best practices and educates others on QI concepts

     

     

     

     

     

    ·         Identifies knowledge gaps and strategizes with education team on ways to close those gaps

     

     

     

     

     

    ·         Conducts training for staff on clinical chart audits, peer review, grievances and appeals, incident reporting

     

     

     

     

     

    ·         Participates in contract oversight activities by performing LHCSA audits

     

     

     

     

     

    ·         Participates in CenterLight Health Services performance improvement projects e.g. HEDIS, IPRO

     

     

     

     

     

    ·         Applies knowledge of regulations pertaining to all CenterLight programs to ensure compliance, seeking guidance as needed

     

     

     

     

     

    ·         Participates in/or chairs committees or work groups

     

     

     

     

     

    ·         Maintains data and submits reports

     

     

     

     

     

     

    Job Qualifications:

    • Licensed and currently registered RN,  proficient in computer programs such as Excel, Word, Power Point. Knowledge of Access or other database programs a plus.
    • Demonstrated team building skills, with excellent verbal and written communication. 
    • Demonstrated ability to manage multiple projects and be flexible.

     

    Education:             BSN, Masters preferred                      

     

    Experience:            4 years of quality improvement experience in long term home health care    

     

     

    Licenses:                Registered Professional Nurse                            

     

     

    Physical:                 Able to travel between regions.
