  • Education and experience: 

    1.        Registered Nurse (RN) licensed to practice in New York State.

    2.        Baccalaureate Degree in Nursing and

    Two years experience in home care as a community health nurse in a Certified   

    Home Health Agency and/or a Long Term Home Health Care Program


         c) minimum of four years experience in home care and

         d) Six credit hours, or the equivalent of education/training in public health and

    Principles of management.



    1.        Demonstrate Strong leadership, managerial and team building qualities in organizing and maintaining home care operations.

    2.        Knowledge of New York State Department of Health Code and Regulations, and New York State Department of Social Services.

    3.        Master's degree in Nursing desirable.

    4.        Vehicle and current New York State driver's license required.

    5.        Basic computer skills required.


    DESCRIPTION OF KEY RESPONSIBILITIES:                                                      


    1.      Assigns and monitor caseload of each member of the health care team by monthly calendar and collaboration with Service Manager.

    2.      Promotes the team concept by establishing joint projects, planning and promoting staff recognition and celebrations.

    3.      Oversees the day-to-day operations of clinical component of HBOC Pathways system for his/her assigned team by utilizing custom reports and competency evaluation.

    4.      Develops, interprets and implements the policies and procedures of the LTHHCP for, and with the members of the health care team by participating in policy and procedure committee and present during staff orientation.

    5.      Supervises, instructs and guides Registered Nurses in the delivery of quality home health care services by monitoring their caseload, documentation compliance, monthly chart review and case conferences.

    6.      Participates in the orientation of the health care team members by establishing orientation calendar, directing sessions and completing field evaluations.

    7.      Establishes and maintains collaborative relationships with all levels of staff including vendor agencies and local DSS offices to accomplish agency objectives by communicating on a regular basis via telephone, facsimile, and E-mails.

    8.      Participates with the Performance Improvement/Education Manager in the planning and delivery of inservice education programs for all members of the health care team by sharing new ideas for educational programs and providing assistance with yearly staff competency, presentations and joint Performance Improvement projects.

    9.      Participates in the survey process of HRA, DOH, JCAHO through preparation field visits with surveyor and document management.

    10. Participates in utilization review, adverse events, performance improvement, professional advisory, marketing/health fairs, revision of job description and other assigned committees of the LTHHCP.

    11. Works in conjunction with the Intake/Screener in preparation for patients to begin services provided by communicating with CRIA staff regarding LTHHCP eligibility by reviewing admission packages and attending pending meetings bi-monthly.

    12. Provides field supervision and competency assessment for the Registered Nurses on his/her team by providing consistent feedback on new and improve skills to enhance job performance.

    13. Performs periodic performance evaluations for all team members, probationary/annually and as needed.

    14. Reviews visit reports, Oasis compliance, summaries and progress notes of team members to monitor adherence to professional standards of practice and agency's policies through 10% of caseload of new admission, Medicare maximization and 120 days re-assessments review.
