  • Interviews callers, assesses all calls from the community and referrers; acts as liaison for the program by marketing the benefits of the program, and coordinates all patient referrals


    Assigns the telephone screening to Leadership for appropriateness in the language spoken by patient; obtains and manages all information related to referral


    Enters patient’s demographic information into HBOC upon being referred; collaborates with Director regarding all referrals on weekly bases and the needs for follow-up.; refers to the Medicaid Coordinator, tracks all information related to Medicare and Medicaid entitlement.; maintains marketing activities log and daily/weekly department statistics, through various software.


    Assists Leadership in creation of graphics and other printed material used for marketing purposes, as needed; coordinates the translation of marketing materials as needed; monitors inventory and coordinates the purchasing of marketing materials, including updating and revising marketing materials/forms.


    Collects and reports accurate program census information for daily/weekly intake statistics; communicate updates on pending referrals with referral sources on a weekly basis; contacts referral sources on a weekly basis to inquire of potential referrals; verifies patient insurance, associated physician NPI # and license #.

    Assigns the screening visits to Leadership, FFS or screening NCM; coordinates all schedules and arrangements for enrollment.


    Refers new case to LTHHCP Medicaid Coordinator for dis-enrollment assistance for managed care; refers new case to LTHHCP MSW to discuss surplus with patient; contacts vendor agencies regarding potential cases early for difficult to serve language request; or for difficult to serve areas.


    Candidate must have at least 1-2 years experience in the health field along with   

    High School Diploma; College Degree preferred. Medical terminology a plus.  Knowledge of excel and PowerPoint a plus.HBOC experience required, computer literate
