Job Details:
  • Education and experience: 

    Registered Nurse (RN) licensed to practice in New York State.

     Baccalaureate Degree in Nursing and

    Two years experience in home care as a community health nurse in a Certified   

     Home Health Agency and/or a Long Term Home Health Care Program

    or

    Minimum of four years experience in home care and six credit hours, or the equivalent of education/training in public health and principles of management.

    Education experience preferred.

     

    Qualifications: 

    Strong leadership, managerial and team building skills.

    Knowledge of New York State Department of Health Code and Regulations, and New York State Department of Social Services; Master’s degree in Nursing desirable.

    Vehicle and current New York State driver’s license required.

    Supervisory experience preferred; excellent organizational and presentation skills.

     

    Monitors caseload of field nursing staff and assess appropriateness of current caseload to level of expertise by use of Pathways reports.

    Interprets and implements the policies and procedures with the clinical staff by arranging and providing educational in-services.

    Supervises, instructs and guides field nursing staff in the delivery of quality home health care services and counsels as needed by competency evaluation, field supervision and monitoring of laptop by using field audit tools and nursing audit tool and nursing audit tool.

    Participates in the orientation of clinical staff; participates with the Performance Improvement Manager and Clinical Information Specialist in the planning and delivery of in-service education programs for clinical staff or on clinical issues and information systems.

    Participates in the Utilization Review, Performance Improvement, Quality Management, Professional Advisory, and other assigned committees of the LTHHCP.

    Provides field supervision for nursing staff annually for competency evaluation and for new equipment or patient issues by doing annual field evaluation, PRN field visits with staff and demonstration of new equipment; maintains and reviews assignment and productivity records and trends for outstanding documentation and/or missed visits of field nursing staff by use of Pathways reports including reviewing, scheduling, assignment completion, transfers done on laptop, date entered and completed.

    Reviews documentation, including OASIS, to monitor adherence to professional standards of practice and agency’s policies; coordinates team conferences to monitor delivery of services to clients and facilitate team-building activities.

    Participates in peer review

    Rotates the 24-hour/7-day/week “on call” administrative backup system
