Job Details:
  • Licensed and currently registered as a professional nurse in New York State.


    Bachelor of Nursing degree required, or matriculated in a program leading towards a Bachelor's of Nursing Degree, Masters in Nursing preferred.


    BCLS Certification; IV Certification


    Working knowledge of RUGS,PRI and MDS 2.0


    Assists in planning, implementation and evaluation of nursing care for residents on his/her neighborhood(s).


    Make rounds to observe residents' needs and the quality of residents' care delivered by staff members.


    Ensures compliance in all areas of clinical documentation through frequent reviews of charts, care plans, NAARS, flow sheets, etc.


    Relates to residents and their families for supportive and health teaching purposes.


    Implements current approaches of delivery of nursing care services on the neighborhood(s).


    Assists with the initiation and accurate completion of MDS 2.0.


    Monitors and evaluates clinical program/project which enhances resident quality of life, e.g., Fall Prevention Program, etc.


    Conducts staff conferences and meetings.  Submits minutes of meetings to Neighborhood Director.


    Provides leadership and acts as a resource person for staff.


    Acts as a change agent by influencing attitudes and promoting good working relationships among staff.


    Participates in determining staffing pattern according to resident needs.


    Participates in orientation of new staff, evaluating performances and makingrecommendations when necessary.


    Supports established goals, objectives, standards for the neighborhood(s) and communicates them to the staff.


    Participates in disciplinary action and counseling; evaluates and documents conferences and actions of staff.


    Ensures that the equipment is in good operating condition in order to maintain a safe working environment.


    Participates in survey preparation and offers recommendation.


    Prepares assigned reports and submits them to the Neighborhood Director/Associate Director of Clinical Services.



    Identifies learning needs of staff and in collaboration with nursing education, assists staff to develop his/her skills.


    Participates in continuous Performance Improvement activities.  Writes and reports findings and recommendation.


    Demonstrates knowledge in labor management issues.


    Monitors lateness, absenteeism of his/her staff.


    Respects the patients/residents rights.


    Performs other related duties assigned by the Neighborhood Director/Associate Director of Clinical Services.  In an emergency, performs other duties necessary for the welfare of the residents or the efficient operation of the facility.
