Interviews callers, assesses all calls from the community and referrers, acts as liaison for the program by marketing the benefits of the program, and coordinates all patient enrollments.
Obtains and manages all information related to referral and enrollment.
Enrolls patients, establishes patient records, and, in collaboration with the Entitlements Coordinator, tracks all information related to Medicare and Medicaid entitlement.
Maintains marketing activities log and daily/weekly department statistics through various software programs.
Assists Manager in creation of graphics and other printed material used for marketing purposes.
Monitors inventory and coordinates the purchasing of marketing materials, including updating and revising marketing materials/forms.
Documents admissions and discharges.
Communicates in a timely fashion (by telephone and through letters as appropriate) all information regarding each referral to the referrer, family, and intake team.
Coordinates all schedules and arrangements for enrollment- related visits to the Day Health Center, DTC, the hospital or the patient’s home.
Documents minutes, and compiles and types information for weekly intake meeting as necessary.
Collects and reports accurate program census information for daily/weekly intake statistics.
Assists Manager with monthly marketing presentation breakfast, including slide presentation and facility tours.
Provides Spanish/English for intake and other CCM staff if applicable.
Interfaces with representatives from state and local government, other health care providers and related organizations to facilitate flow of information.All other duties as assigned by Supervisor
