- RN license. 5 years of clinical experience. Knowledge and education, including but not limited to the following; clinical expertise, consultation skills, teaching/learning principles, research process, change theory, management skills, financial analysis and group process. CPR required. Case Management or adult Med/Surg experience preferred Case Coordinator I is accountable for the clinical and financial outcome of care for an assigned caseload. The Case Coordinator functions as an advanced practitioner in the role of clinician, educator, researcher, manager, and advocate. The Case Coordinator facilitates the care of patients during their hospitalization by managing, coordinating and monitoring resources to achieve optimal level of independence and health for patients and families; clinical outcomes; utilization; service, and financial goals by performing the roles of assessor, planner, facilitator and advocate. The Case Coordinator uses Continuous Improvement (CI) principles.
