Case Coordinator I is accountable for the clinical and financial outcome of care for an assigned caseload. The Case Coordinator functions as an advanced practitioner in the role of clinician, educator, researcher, manager, and advocate. The Case Coordinator facilitates the care of patients during their hospitalization by managing, coordinating and monitoring resources to achieve optimal level of independence and health for patients and families; clinical outcomes; utilization; service, and financial goals by performing the roles of assessor, planner, facilitator and advocate. The Case Coordinator uses Continuous Improvement (CI) principles.
*RN with current licensure.
*5 years recent clinical experience with 3 years of supervisory experience preferred - charge nurse or team leader.
*Communicates well with others.
*Knowledge and education, including but not limited to the following; clinical expertise, consultation skills, teaching/learning principles, interpersonal skills, research process, change theory, management skills, financial analysis and group process.
*Case Management or adult Med/Surg experience preferred.
