Provides leadership and consultation to patients, families, nurses and other health care providers to effect patient care outcomes. Utilizing established guidelines, assesses patients who have completed active therapy and are being followed on a regular basis for any evidence of disease activity. Evaluates patients who develop acute problems when physician is not available. Prescribes medications according to treatment plan, and refers patients according to guidelines. Coordinates the day-to-day operations and fulfills the responsibilities of the Clinical Coordinator of the Department. Coordinates the case management of patients being followed by the department. Graduate of NLN approved Nurse Practitioner Program (certified in speciality). Masters degree in Nursing requried. Minimum of 2 years of nursing experience required. Experience in Nurse Practitioner role preferred. Current Vermont RN license with evidence of eligibility for ARNP required and be Certified in Healthcare Providers CPR.
