The Registered Nurse plans patient care for a defined group of patients, delegates components of the plan of care, and delivers care based on the standards of care of assessment, diagnosis, outcome identification, planning, implementation, and evaluation.
Essential Duties and Responsibility Levels:
A. Using appropriate assessment parameters, collects in a systematic and ongoing fashion, pertinent data about the patient, involving as appropriate, the patient, significant others, and other healthcare providers.
B. Derives the nursing diagnoses from the assessment data, validates them when possible with the patient, significant others, and other healthcare providers, and documents the diagnoses in a manner that facilitates the determination of expected outcomes and of a plan of care.
C. Identifies realistic, expected outcomes individualized to the patient that are mutually formulated with the patient, significant others and other healthcare providers.
D. Reflecting current nursing practice, develops a plan of care that prescribes interventions to attain expected outcomes.
E. Implements the interventions that are consistent with the established plan of care, the interdisciplinary treatment plan, and available economic resources, according to the patient's age and/or stage of development, from adolescent through geriatric.
F. Evaluates the patient's progress toward the attainment of outcomes, making appropriate changes in the plan of care as the patient's condition changes, seeking appropriate responses from physicians and other care providers while preventing and detecting errors.
