Guggenheimer Health and Rehabilitation Center is a 130-bed health and rehabilitation facility located in the historic Diamond Hill district of Lynchburg, VA. Our focus is to provide you with the care you need to recover and return home, at your pace, after illness, hospitalization or surgery. We offer physician-directed skilled care nursing services and rehabilitation that will help you or your loved one regain strength, endurance, mobility and independence.
The Licensed Practical Nurse at Fairmont performs nursing care by planning, coordinating, providing, and evaluating care for a group of assigned residents to include the following:
· Medication administration
· Treatments
· Assessments
· Leads, communicates and collaborates with physicians, residents, families and staff to plan resident care.
· Creates and maintains an environment that protects residents from harm.
Educational Requirements
Graduate of a school of licensed practical nursing
License Requirements
Virginia LPN license
Preferred Experience
Long-term care LPN experience preferred.
