    The Care transitions nurse plays a key role within a multiple disciplinary psychiatry team. This dynamic individual will be responsible for evaluating admissions, promoting successful transitions and identifying, coordinating and communicating barriers. This candidate is responsible for the clinical aspects of determining appropriate discharge plans for patients and their families in the acute care, complex setting. This member of the care transition team will be responsible for engaging, partnering and advocating for the patient population through healthcare and community support needs. The ideal candidate will have strong facilitation and organizational skills, be an excellent communicator, have knowledge of state, federal and regulatory requirements. This position works collaboratively with multiple other disciplines throughout the organization and community. 


    Required Education

    Registered nurse.
    Maintain Required Competencies for Career Ladder


    Preferred Education



    Required Experience

    3 years' experience in acute care nursing.
    Computers skills a must as well as excellent communication and the ability to work collaboratively with other disciplines.


    Preferred Experience

    Working knowledge of d/c planning, LTC, or Medicare regulations desirable


    Certification/Licensure Requirements

    Active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse.


    Preferred Certification/Licensure

    ACMA Certification
