Hospice Admissions RN

The Hospice Admissions Registered Nurse is responsible for receiving and processing referrals to the Hospice program in a home care setting. Explaining the Hospice philosophy and program; obtains necessary medical information; obtains certification of the terminal illness (verbal and written) from the primary physician and coordinates screening/admission visits.
· Current and valid NY State license to practice as a Registered Nurse
· 1 – 2 years of post-licensure experience in an acute care setting
· Good computer and Electronic Medical Records skills.
· Professional and compassionate communication skills
· BSN and prior experience in home care or hospice preferred
