Job Summary: The Director of ACO Operations will be accountable for a broad range of initiatives and programs that will be centrally managed across Central Maine Healthcare. The Director will oversee a team of administrative and clinical individuals who will support activity across all sites and the full continuum of care. The Director will be provided clinical direction and support by the ACO Medical Director and/or the Chief of Population Health. Specific oversight may include, but is not limited to, oversight of the following efforts: Care management operations, including chronic disease, transitions of care and high risk patient programs, population health and quality improvement programs and risk coding optimization across the delivery system. The Director will guide the team in partnership with CMH practice leaders, providers external care partners. The Director of ACO Operations will be accountable for programs and initiatives to drive performance across all applicable payer quality programs. The Director will be responsible for defining a system-wide framework for risk coding and ensuring design and implementation of programs that ensure providers are assigning the appropriate risk codes for their patient panels.
Educational Requirements: Bachelor's degree required. A clinical license such as an RN is preferred. A Master's Degree in Business Administration, Health Care Management/ Administration is preferred.
Experience Requirements: Requires at least 7 or more years in healthcare with progressive responsibility over care management and population health management programs.
