Job Summary: The Patient Flow Coordinator is responsible for ensuring effective and efficient patient throughput across all CMMC Clinical Departments. (S)he obtains patient information in order to assign, control, and log appropriate service accommodations for patients admitted to the Medical Center and coordinate special transfers according to room availability and admission approval from other medical facilities. The Coordinator is responsible for optimizing patient throughput taking into consideration balanced staffing patterns for the inpatient units and the Emergency department while facilitating patient bed placement. The Coordinator will collaborate with the Float Pool Associate Nurse Leader to assure adequate staffing levels are maintained, on all shifts daily, based on acuity, census and unit activities, The Coordinator will work in collaboration with personnel from the emergency department, referring physician offices, surgical sites and inpatient floors, to facilitate the throughput of all patients, by managing patient bed placement using the hospital identified criteria.  The Coordinator will collaborate with physicians when needed, to ensure quality patient care by focusing on "correct patient placement the first time".  This includes receiving requests for admissions and working with the nursing units and leaders to identify the best available bed placement based upon patient needs, level of care requirements, and infection control principles. The Patient Flow Coordinator role is critical to the patient experience and the physician's perception of the service's we provide.  As such, the Coordinator helps to establish corporate credibility and support current business.

This position works under limited supervision and requires strong organizational and planning skills, decision making within the scope of the position's responsibilities, and the use of discretion and good judgment.  Decisions beyond the scope of responsibilities require the review and approval of the Director of Capacity Management.  The Coordinator will demonstrate the ability to set goals and judge results in accordance with the highest standards; care with sensitivity and respect; advance care through development of new ideas and technology; promote teamwork to achieve CMMC's mission; and understand and exceed customer expectations.

Key Duties and Responsibilities:

1.    Manages multiple tasks simultaneously, able to quickly prioritize/triage work assignments.

2.    Assesses hospital capacity capability, service options, resolve problems, determine patient needs, and identify options to meet patient's needs.

3.    Establishes positive relationships with stakeholders and staff at all levels. Intermediate computer, spreadsheet, & database skills.

4.    Collaborates and negotiates with all levels of hospital personnel, physicians, patient/families and outside agencies. Ability to resolve conflict, including situations where there may be conflicting priorities.

5.    Prioritizes and completes duties in a timely and organized manner; Ability to work collaboratively and in a positive manner with others; Ability to perform work according to CMMC policies and procedures, professional standards of care, and applicable laws.

6.    Demonstrates flexibility and creativity in response to unexpected changes in work load/staffing; care with sensitivity and respect.

7.    Communicates special needs of new admits to staff to ensure a smooth transition

8.    Manages the admission process by maintaining updated bed availability and facility services information at all times. 

9.    Maintains tracking and credentials and departmental data associated with staffing.

10.  Updates and monitors daily schedules.

11. Arranges for supplemental staff when necessary in coordination with clinical leaders.

12. Maintains poise in stressful situations and continually exhibit outstanding public relation skills.





  1. Maintains current knowledge of admission guidelines and policies and procedures pertaining to patient placement into beds.




  1. Provides proactive management of admission requests.
  2. Manages the timely turnaround of patient beds. Communicates capacity issues and resolution to nursing, physician leadership, and administration.
  3. Prioritize, coordinates, and expedites placement of patients using primary diagnosis, patient placement Algorithium, and understanding any isolation or extenuating circumstances.




  1. Serves as an advocate of effective and courteous interdepartmental communication to facilitated house-wide movement of patients or movement between units and regional campuses.




  1. Monitors and maintains bed tracking board to display  bed availability, in-house transfers and anticipated discharges by making periodic, frequent rounds on patient floors




  1. Collaborates with EVS to ensure the highest priority for the cleaning of beds.




  1. Assess critical situations and works collaboratively with Unit Leaders ensuring appropriate bed and staffing utilization.




  1. Collect complete and accurate patient referral data




  1. Serves as a liaison to Case Managers,  and Medical Staff




  1. Participates in professional development to assure competency in skills and knowledge necessary for job requirements.




  1.  Participates in the evaluation and initiation of the admission processes of ongoing performance improvement.




  1. Provides accurate and timely communication, both written and verbal regarding projects, data, patient information, and in response to internal and external customers.




  1. Identifies when assistance is needed and demonstrates the self-confidence to request as needed.




  1. Demonstrates being self-directed.  Identifies program needs and searches out solutions to discuss with Connect Referral Specialist.

Minimum Requirements  


·                    Current state Registered Nurse Licensure  with 3-5 years of clinical experience in complex settings

·                    Bachelor's Degree required, BSN preferred

·                    Five years' experience in the use computer software applications, with Intermediate Microsoft Office Skills

·                    Current American Heart Association Healthcare Provider BLS Certification required


·                    Prior healthcare industry experience in similar role highly desirable

·                    Ability to work independently

·                    Highly organized, dependable, and multi-tasker

·                    Demonstrate hands-on computer experience (Microsoft Office, web applications, scheduling tools among others)
