The Diabetes Specialist performs Diabetes Self-Management Education and Training for inpatient and TCU/Rehab patients through assessment, evaluation, education, medication management, planning, organizing and implementation of care plans, and discharge planning. Collaborates with the Glucose Control Team, Attending Provider and Nursing staff. Provides staff in-services and assists with Diabetes Program, classes and outpatients as needed.
DUTIES AND RESPONSIBILITIES:.
· Ability to develop diabetes education programs and activities that support the mission, philosophy, and framework of Confluence Health.
· Ability to apply advanced adult & pediatric learning principles in the design, implementation, evaluation, and oversight of diabetes education for acute care and TCU/Rehab patients.
· Teach self-management education, including diet, exercise, self-monitoring of blood glucose, insulin injection and medication adherence.
· Provide Insulin Pump guidance for staff and patients, acquire pump certifications (major brands).
· Ability to provide professional medical services within the scope of training and responsibility in compliance with applicable laws, regulations and Confluence Health policies, procedures and protocols.
· Identify opportunities to optimize drug therapy and plan for discharge.
· Participate in studies as needed.
· Participate in the Diabetes Quality Committee.
· Provide staff education and in-service via classroom presentation, counseling, and excellent communication skills as needed.
· Develop, edit, proof read and evaluate educational materials as needed.
· Ability to function independently and as a member of a cross-functional team.
· Ability to organize and set work priorities in a multi-task environment.
· Other duties as assigned.
· Must be proficient with Windows navigation, Microsoft Word, Excel and Outlook.

KNOWLEDGE AND TRAINING:
· Minimum Bachelor's degree in healthcare related field such as Registered Dietitian, Registered Nurse, Registered Pharmacist, PA-C or ARNP
· 2-3 years of experience, with an emphasis on Diabetes Management and Education.
· Certified Diabetes Educator or BC-ADM, or is eligible to become a CDE or BC-ADM within 12 months of hire.
Has frequent contact with physicians, staff, and executive leadership. Has extensive contact with confidential information.
INITIATIVE AND JUDGEMENT:
Works with moderate to little supervision.
Moderate amount of time requirements outside normal working hours.
