Position can sit in Bedford or Lebanon.

Engages and manages relationships with partners, Federally Qualified Health Center's (FQHCs), Community Health Centers (CHCs), Health Systems Community Practices, Insurers and Employers.  Implement at least two evidence-based interventions and supporting strategies with each partner organization, evaluate what interventions and processes were successful and identify the challenges in order to increase their colorectal cancer (CRC) screening rates.

1.    Recruits, contracts and builds relationships with partner leadership teams that are committed to increasing their CRC screening rates through the implementation of evidence based interventions.

2.    Consults and facilitates with leadership and assigned personnel of key partners on activities that will lead to the outcomes that are outlined in the New Hampshire Colorectal Cancer Screening Program (NHCRCSP) work plan and logic model.

3.    Conducts evaluation of interventions and processes to identify what is successful and what are the challenges that require additional intervention.

4.    Act as a resource and mentor to other staff, giving input into performance appraisals and assisting in the interview process, as assigned.

5.    Utilizes known resources such as the Community Guide, American Cancer Society (ACS) toolkit, and develops unique NHCRCSP resources for use by partners.

6.    Consults with partners to identify, recruit and maintain at least two internal champions; one being a medical provider.

7.    Supports training of internal champions and meets with them on a regular basis to provide expertise, ongoing feedback regarding progress toward goals, and address challenges and successes.

8.    Sets goals for percentage increase for coming months and years based on current CRC screening rates. 

9.    Measures programs' progress through analyses of CRC screening rates and provides results and recommendations for improvement as appropriate.

10. Assists partners in developing a uniform, system-wide CRC screening policy consistent with published guidelines.

11. Provides expertise for supporting strategies such as utilizing health informatics to support quality improvement, and professional development training on guidelines and quality.

12. Assists partners in reducing structural barriers to increased CRC screening rates. 

13. Provides access to small media materials that can be customized using Center for Disease Control and Prevention (CDC), and ACS materials.

14. Participates in developing campaigns for the marketing of NHCRCSP programs and services in conjunction with the D-H Public Affairs and Marketing team.

15. Prepares and distributes reports to NHCRCSP and partners' leadership.

16. Explores and develops use of social media to meet NHCRCSP goals.

17. Performs other duties as required or assigned.


Minimum Qualifications:

ü  Bachelor's degree and three (3) years of experience in consulting with health systems or employers required. 

ü  RN, NP, PA, SW (with current NH licensure) or Master's in Healthcare preferred.

ü  Expertise with the Evidence Based Interventions from the CDC Community Guide is preferred.

ü  Experience with Microsoft word and Power Point.

Excellent communication skills and demonstrated ability to work effectively on a team with strong relationship-building skills.
