Collaborates with hospital nursing and case management staffs, facilitating referral and discharge of prospective homecare/hospice patients. Makes visits to hospitalized homecare patients, assisting with pending discharge, and resumption of homecare services. Provides education to customers providing information and educational resources as requested. Provides education to hospital staff regarding qualifications and guidelines of homecare and hospice.
*Must be a Registered Nurse licensed in the State of Illinois.
*Prior home care experience required.
*Excellent communication skills. Must be able to understand and maintain the confidential nature of the information handled and discussed. Ability to work independently and set priorities while understanding the importance of complete, accurate and timely information. Knowledge of homecare and hospice regulations, qualifications, guidelines and available appropriate services.
*Must be able to walk, stand, stoop, kneel and assist in lifting patient; able to assist patients and/or adjust equipment at floor level; able to reach, change and read IV's 5 feet from the floor unaided; able to support persons (possibly 200 pounds or more) in emergency situations; able to move quickly from place to place on occasion, including from floor to floor in emergency situations; able to utilize fine motor control in hands for tasks such as medication preparation and administration; able to hear to take blood pressure and to hear breath, heart and bowel sounds. Must possess visual acuity to meet visual demands in charting and operating equipment.
*Must be able to demonstrate problem-solving techniques in unique situations. Must be able to work under stress and to adapt to changing conditions.
*May provide direct patient care involving the potential for frequent exposure to blood and body fluids.
