  • Position Summary
    • The Licensed Nurse Assistant will ensure efficient direct patient care both adult and pedi as assigned by licensed nursing staff. Duties will include, but not limited to activities of daily living, vital signs, Intake & Output, ambulating patients, charting on flow sheets, and transportation. She / he demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on the Medical/Surgical Unit. The LNA must demonstrate knowledge of the principles of growth and development of the life span and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age, specific needs, and to provide the care needed as described in the unit’s departmental policies and procedures.
  • Qualifications
    • Must hold a New Hampshire LNA license.
    • Current BLS certification
    • High School Graduate or equivalent GED.
    • English speaking and writing skills required.
    • Demonstrate ability to communicate changes, i.e. patient status and significant other issues, to licensed nurse.
  • Essential Functions of the Job
    • Required skills and competencies. Has competency checklist been completed with all competencies met (i.e. job skills / knowledge, equipment knowledge)?
    • Under the supervision of an RN or LPN provide safe, organized and appropriate care to patients assigned. This includes personal hygiene, nutrition and elimination, ambulation and movement, I & O’s, vital signs, weights, chem sticks, assist with admissions / discharges, and assist patients to attain and maintain their optimal functioning independence.
    • Communicate with RN/LPN throughout shift as to completeness of total care. Make suggestions in a considerate and constructive manner.
    • Provide safe and clean environment for patients involving housekeeping concerns: (i.e. patient rooms neat and clean), call bells within patient’s reach, adaptive equipment; and special clothing.
    • Provide patient care activities in a respectful manner to patient’s rights and privacy. Provide emotional support with appropriate responses and techniques used to preserve patient dignity.
    • Offer comfort and rest periods between activities as needed. Provide oxygen as directed by team leader. Assist with transports to other departments.
    • Be available and willing to assist during emergency situations as directed by RN/LPN.
    • Maintains a good working relationship with all levels of personnel.
    • Under the supervision of the nurse, assesses patients and family needs to ensure completeness of care. Identifies potential risks for patient injury and implements risk reduction strategies with the RN/LPN.
    • Under the supervision of the nurse, provides patient care and demonstrates the skill and judgment necessary to carry out appropriate care.
    • Under the supervision of the nurse, documents accurately on I & O, Patient Care Flow Sheet, TRP Sheet, and Vital Signs Sheets.
    • Completes patient care assignments within assigned shift.
    • Maintains patient privacy and dignity.
    • Presents a neat appearance following Weeks Medical Center Dress Code policy.
    • Aware of own actions that may affect patients, families, visitors, or other employees.
    • Helps provide a clean patient environment, restocking as necessary before end of shift.
    • Expresses disagreements through proper channels and in an appropriate and professional manner.
    • Manages time effectively to ensure that deadlines are met and that personal priorities support organizational objectives.
    • Performs all other duties as assigned.
