The role of the Care Coordinator is to provide services and referrals to patients in order to ensure optimal clinical, financial and quality outcomes during the acute hospitalization. The Care Coordinator utilizes professional clinical nursing skills and experience to assess admission and continued stay appropriateness while monitoring the care plan to ensure timely delivery of care and discharge planning. The Care Coordinator will proactively identify patients with post-discharge needs, and collaborate with the patient/family, medical team and payor to coordinate the discharge plan.
Requires RN degree and a current CA license. Requires a minimum of 3 years of clinical experience in an acute hospital setting and a minimum of 2 years of case management experience in an acute hospital setting or IPA/Medical Management group. Good computer skills a must as well as excellent working knowledge of Interqual, Medicare, MediCal, managed care and other insurance plans.
