    We are currently looking for a FLEX Care Coordinator to join our team!

    *Based on experience, may be brought in as a Discharge Planner


    Provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.  Accountable for the care, coordination, and discharge planning of patients.  Integrates the social service function into patient care.  Coordinates hospital activities and ensures communication flow amongst all members of the interdisciplinary, health care team.  Ultimately, acts as a patient advocate to enhance quality, satisfaction, and promote continuity of care from admission to post acute services. 


    Licensed Social Worker or Registered Nurse

    3 years of experience in Nursing and/or Social Services

    Must be a licensed RN or Social Worker: Associate RN with appropriate experience, licensure, and education to adequately perform the job responsibilities and meet the needs of the patient population will be considered. BSN preferred OR BSW with appropriate experience, licensure and education to perform the job responsibilities and meet the needs of the population

    Prefer hospital experience in Quality Management, Case Management, Discharge Planning or Utilization Review.

    Knowledge of disease management and principles of wellness required.

    Specialized/formal case management training or certification is preferred

    Excellent communication skills

    Use of computer for data entry and retrieval is required to perform job duties daily 

     Flex Expectations

    §  FLEX employees will be considered full time employees with a guaranteed minimum schedule of 12 hours per week. 

    §  Expected to be available to work 12 to 40 hours per week.

    §  May receive schedule changes/adjustments with a 48 hr. notice

    §  Must work when scheduled. 

    §  FLEX employees will be eligible for all full-time benefits offered by EAMC as long as guidelines/expectations are met.

    §  ETO time will continue to accrue based on actual hours worked.  LTS will accrue as a full-time employee. 

    §  FLEX staff may refuse a schedule assignment only 5 times within a year before losing FLEX staff status. Upon the fifth refusal, the employee will lose their full-time benefits and be converted to per diem status. (Each refusal shall be documented in Kronos.)

    §  FLEX employees will follow the attendance guidelines outlined in the department attendance policy as it relates to attendance, tardies and call-ins.

    §  FLEX staff may request to be scheduled extra day/hours in addition to their core schedule.  All additional requests must be approved by management. 

    §  FLEX staff may be assigned to other units in which they are competent if they are not needed in their home department.
