The work involves developing, monitoring and coordinating patient care services for the Surgical and/or surgical sub-specialties services of the Erie County Medical Center Corporation. This is a registered professional nursing position. The incumbent utilizes the case management model, focusing on an interdisciplinary team approach to promote quality patient care while achieving desired patient outcomes in a time-efficient, cost effective manner. Work is performed under the general supervision of higher-level professional nursing administrator. Supervision is not a function of this position. Does related work as required. TYPICAL WORK ACTIVITIES: Provides for the comprehensive management of each patient’s transition from admission through discharge; Assesses patient’s medical/psychosocial history and tracks major sequence of events during current hospitalization; Introduces self to patient/family to explain the role of the Nurse Case Manager and assess home support; Collaborates with interdisciplinary team to develop and implement clinical pathways for specific diagnoses and desired patient outcomes; Coordinates discharge with discharge planner developing measurable goals with the patient/family; Accompanies and assists physicians on daily rounds; Consults with physicians on patient status and progress toward expected outcomes and strategies to achieve desired outcomes; Identifies, analyzes and takes corrective actions for patterns of variance that emerge within case management plan; Consults with appropriate parties on executing hospital strategies for balancing quality care and cost according to hospital database diagnosis-related group (DRG) experience Individualizes the case management plan based on the patient’s social and economic data, family resources, functional abilities, knowledge needs, potential risk factors and complications and special concerns; Monitors and evaluates patient’s progress on a daily basis; revises case management plan as needed; Monitors charts and nursing documentation; Participates in interdisciplinary meetings. FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of the principles and practices of professional nursing; through knowledge of current nursing practices and techniques used to provide total patient care in surgical services; through knowledge of diagnostic related groups (DRG); good knowledge of medical case management planning, principles and techniques; ability to establish and maintain both professional and personal relationships with patients, patients’ family and Medical Center staff; critical thinking skills; patience, compassion, physically capable of performing the essential functions of the position with or without reasonable accommodations. MINIMUM QUALIFICATIONS: A) Graduation from a regionally accredited or New York State registered college or university with a Master’s Degree in Nursing and four (4) years of acute hospital nursing experience, including one (1) year of acute surgical nursing experience; or B) Graduation from a regionally accredited or New York State registered college or university with a Bachelor’s Degree in Nursing and five (5) years of acute hospital nursing experience, including one (1) year of acute surgical nursing experience; C) An equivalent combination of training and experience as defined by the limits of (A) and (B). SPECIAL REQUIREMENT: Possession of a license and current registration to practice as a Registered Professional Nurse in the State of New York. NOTE: Verifiable part-time and/or volunteer experience will be pro-rated toward meeting the full-time experience requirements.
