Job Details: The work involves performing social case work services at the AIDS Center, Erie County Medical Center. This is a social work position responsible for providing information and assistance to both impatients and outpatients of the AIDS Center. The incumbent provides patients and their family with counseling, education, support and community services information. Work is performed under the direct supervision of higher level professional social work staff. Supervision may be exercised over lower level social work and/or clerical staff. TYPICAL WORK ACTIVITIES: Assists with case management services provided by social work staff of the AIDS Designated Center under the direction of the Social Casework Manager (CM Unit Coordinator); Provides a mechanism within a community to assure early medical interventon for the HIV positive person; Provides HIV counseling, education, emotional support, reinforcement, encouragement, advocacy and guidance for persons living with HIV infection; Assists in the AIDS Center Clinical and Case Management staff with the follow-up of patients who are non-compliant with medical care through Immunodeficiency and Women''s HIV Clinic; Participates in Case Conference and Case Management Review of both inpatients and outpatients receiving services through the AIDS Center; Conducts home visits for patients who have missed clinic appointments to avoid lapse in medical services and assure optimal health status; Assists with the evaluation of patient needs through the outreach mechanism to assist in the development / update of the patient service plan; Increases patient awareness of their HIV status throughout the continuum of illness; Motivates patients to take advantage of community resources to enable empowerment over their HIV status; Completes required paperwork , documentation and daily activity logs in conjunction with daily work. FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES, AND PERSONAL CHARACTERISTICS: Good knowledge of modern pronciples and practices of social caseworker; good knowledge of the social , emotional and legal problems faced by AIDS patients and their families; working knowledge of social work standards specially relating to AIDS patients and their families; ability to work with AIDS patients and their families in a cooperative and beneficial mariner; ability to maintain confidentiality in all matters; skill in counseling AIDS patients and their
families; tact; dependability; initiative; physical condition commensurate with the demands of the position.

MINIMUM QUALIFICATIONS:

A) Graduation from a regionally accredited or New York State registered college or university with a Bachelor's degree and one (1) year of social casework experience in a hospital or health care setting; or:

 

B) Completion of a minimum 60 semester credit hours from a regionally accredited or New York. State registered college or university and three (3) years of social casework experience in a hospital or health care setting; or:

 

 C) An equivalent combination of training and experience as defined by the limits of (A) and (B).
