The work involves performing medical social work within the Department of Social Services or the Erie County Medical Center Corporation and in conjunction with outside hospitals and agencies. The work is performed under the general supervision of a Supervising Medical Social Worker or the Director of Social Work and Discharge Planning. Does related work as required.
TYPICAL WORK ACTIVITIES:
Interprets social factors in individual cases to physicians;
Utilizes community medical and health resources;
Assists in the determination of medical, social and psychological needs and maintains records in individual cases;
Evaluates and interprets social, psychological and medical factors to determine the eligibility of the client for medically indicated services such as transportation, recipient restriction program, pharmacy services and other community services;
Evaluates and interprets the social/medical factors relevant to employability and/or eligibility of the client for disability and other benefits;
Makes referrals for medical, psychological and social programs;
Acts as liaison between staff and community groups and individuals in medical social matters;
Works with families when indicated to help strengthen the client’s support system;
Establishes and maintains effective working relationships with medical and allied professionals and other agencies in the community;
Participates in developing medical social work policies and procedures by observing client needs and making recommendations at conferences;
Interprets medical social casework to staff and community.
FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Good knowledge of Federal, State and local laws affecting medical and health programs; good knowledge of practices and procedures of medical social case work the social factors effecting disease; good knowledge of high quality medical care; working knowledge of the proper use of diagnostic information; working knowledge of medical ethics; ability to organize and prioritize work; ability to establish and maintain an effective working relationship with clients and their families; ability to interpret the medical program of the agency to the health agencies of the community; imagination and creativity; sound professional judgment; initiative; tact; resourcefulness; physically capable of performing the essential functions of the position with or without reasonable accommodation.

1) Section 424-A of the Social Services Law requires that local social services district to inquire whether the applicant is the subject of an indicated child abuse or maltreatment report on file with the State Central Register of Child Abuse and Maltreatment. All potential employees for this position will be requested to sign the necessary clearance form prior to being advised that they will be hired. 2) Possession of a license and current registration as a Licensed Master Social Worker (LMSW) issued by the New York State Department of Education at the time of appointment.
