The work involves responsibilities for managing casework activities to ensure that infants and toddlers with developmental delays or disabilities receive appropriate services when determined necessary through the evaluation process. The incumbent assists in enhancing the capacity of the family to meet the developmental needs of their infant or toddler through accessing community resources, as well as making sure they understand the importance of advocating on their own behalf. Work is performed under the general supervision of a Senior Service Coordinator of Early Intervention Services, who reports to the Director of Services for Children with Special needs. May supervise lower level technical or clerical personnel. Does related work as required.
TYPICAL WORK ACTIVITIES: Provides information and referral to families of children with suspected developmental delay; Assists with the scheduling and receiving a multi-disciplinary evaluation for a suspect child; Oversees the case once it has been opened to ensure that services are being provided and the family''s needs are being met; Assesses families'' needs to determine family strengths and to identify needs and required services; Tracks the amount of time spent servicing the family and enters information into the data system, which will record productivity; Maintains activity reports, case records, and other documentation, submitting written reports as required; Facilitates and participates in the development, review, and evaluation of the Individual Family Service Plan (IFSP); Processes authorizations for services on IFSP with data entry; Makes arrangements and oversees the services contained in the IFSP; Monitors the services contained in the IFSP to ensure timely and appropriate delivery of services; Fosters and maintains a good working relationship with families, Medicare, health, social service, and other community agencies; Represents Erie County and Early Intervention at meetings held in the community; Attends meetings and in-service programs to keep informed on the latest developments; Assists family with transition from Early Intervention to Committee on Preschool Special Education (CPSE), if determined, as necessary.
FULL PERFORMANCE KNOWLEDGES, SKILLS ABILITIES AND PERSONAL CHARACTERISTICS: Good knowledge of the principles of social work as related to infants and toddlers; Good knowledge and understanding of infants and toddlers who may be eligible for early intervention services; State and Federal regulations pertaining to the Early Intervention program; principles of family-centered services; nature and scope of services available under the EIP, and system of payments for services in the State; Ability to establish and maintain relationships with their families and service providers; Ability to interview, assess, establish, and implement case plans on behalf of the infant/toddler and their families; Good organizational skills as well as time management skills; Initiative; Confidence in functioning autonomously; Reliability; Dependability; Tactfulness; Resourcefulness; Good judgment; Emotional maturity; Ability to assert self-regarding decisions made during meetings; Physically capable of performing the essential functions of the position with or without reasonable accommodation.
MINIMUM QUALIFICATIONS: a) Graduation from a regionally accredited or New York State four (4) registered college or university with a Bachelor''s degree in Social Work, Human Services, Child Care, Early Childhood Education, Elementary Education, Child Development, Nursing, Business, Finance or related field; or: b) Graduation from a regionally accredited or New York State two (2) year college with an Associate''s degree in a human service or health field; and at least six (6) months experience in service coordination; or: c) Two (2) years experience in service coordination activities as delineated in Section 69-4.4 of Early Intervention Regulations; or: d) An equivalent combination of training and experience as defined by the limits of (a), (b) and (c).
SPECIAL REQUIREMENTS: New York State driver''s license required. Since the position requires substantial travel, candidates must demonstrate the ability to meet transportation needs of the job in carrying out their responsibilities in any location to which travel is required, including some not accessible by public transportation. Section 424-a of Social Service Law requires the local social service district to inquire whether the applicant is the subject of an indicated child abuse or maltreatment report on file with the State Central Register of Child Abuse and Maltreatment. NOTE: The age limits for infants and toddlers for early intervention is defined as being from birth to three (3) years old. NOTE: Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full time experience requirement.
