Education:

Minimum: Graduate of an Accredited School of Registered Professional Nursing

Preferred:  BSN,

 

License/Certifications: Current NYS License to Practice as a Registered Professional Nurse.

Experience:

Minimum: Five years health care related experience. Preferred:  Prior Care Management or Case Management experience in hospital setting

Other:

Minimum: PRI and/or Screen certification or intent to pursue within 6 months.  Must be able to travel to multiple locations on any given day as requested.  Demonstrated ability to handle confidential information with discretion and ability to deal with the public in a professional and courteous manner.  Ability to meet deadlines, manage multiple priorities and enhance the spirit of teamwork through effective role modeling.  Computer literacy required with word processing, spreadsheets

Preferred:  Experience with Microsoft Office products including  word, excel

Responsibility Summary:
Must be able to apply interview techniques to adequately and comprehensively assess, plan, implement, coordinate, monitor, and evaluate options and services to meet an individual's health needs. Must be able to interact with all members of the health care team and assist with the implementation of the care plan through a multidisciplinary care planning process. Must be able to interact effectively with all payers to ensure proper reimbursement for services rendered. Through communication and available resources promote and provide cost-effective and cost-efficient strategies which maximize quality care and cost effective outcomes within the current healthcare environment.  Provides hospital case management / utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.  Coordinate the integration of the social service function into patient care
