  • We have a part-time position, 20-hours per weekend or every other weekend, for a Revisit RN in our Home Health Department.  If you enjoy working independently, and have a passion for excellence, this may be the position for you. The hours are 9 a.m.-5:30 p.m. Fridays, 8 a.m. to 4:30 p.m., Saturdays, Sundays, and Mondays and also being on call from 4:30 p.m. Friday-8 a.m. Monday.   The responsibilities of this position include: seeing patients in case manager's case load, and providing care to Home Health patients including infants, children, adolescence and adults (including geriatrics) in their homes. Nursing care includes physical assessment, procedures/skills, administration of medications, treatments and implement interventions set by doctor’s orders to improve outcomes and rehabilitation potential. Individual will supervise clinical work completed by the Home Health Aide and work collaboratively with RN Revisit, MSSW, OT, COTA, PT, PTA, ST, Office Coordinator, Home Health Dept. Associate and other personnel as appropriate. Must complete timely documentation after each visit including procedures, medication reconciliation, falls risk and re-hospitalization risk, aide assignment and discharge planning. 


    Qualified candidates must be Wisconsin licensed RN, with one-year of Medical/Surgical, Home Health, or SNF experience. Home Health experience preferred. This position required a valid Wisconsin Driver's license; auto insurance coverage on vehicle used to perform job; CPR and glucometer certification within department guidelines; and re-certification as required. Must have computer knowledge and hold a two-year Associate degree or 3-year diploma or Bachelor degree in nursing. Must be able to communicate directions to patients, family members, CNA's, dept. associates and other personnel and be able to describe patient condition to physician, other medical professionals, patients and family members. Individual will offer comfort to patients and be able to recognize and respond to both verbal and non-verbal cues and communications
