High school graduate or equivalent required


Good written and organizational skills. Demonstrate ability to set priorities and organize tasks in a timely manner. Ability to work independently in a fast-paced and rapid changing environment


  One year experience in assisted living facility preferred.




DEFINITION: The Personal Care Assistant (PCA) performs routine duties, in caring for residents which are supportive and complementary to nursing practice.



  1. Actively participates in the resident's plan of care and assists in implementing care intervention at residents level of care.
  2. Upon resident admission, assists licensed personnel with data collection, through basic physical appearance, vital signs, weights, and documents accordingly. Continues basic assessments during routine resident care on a daily basis, reporting change of condition to Facility Director/Physician/and family members in a timely manner.
  3. Charts clearly and concisely in a timely manner, according to policy and procedure. 
  4. Provides a clean and safe environment for all residents.  Assists residents to their individual level of capability, their cleanliness, grooming, nourishments, rest, activity and elimination.  Ensure teeth brushing daily.
  5. Demonstrates ability to assess emergency situations and take appropriate action.
  6. Reports to physician and resident's responsible party change in condition in a timely manner and performs all necessary documentation. This includes calling families when a resident falls.
  7. As assigned, completes any resident care responsibilities or assignments left from previous shift and reports any incomplete assignments to the next shift.
  8. Makes routine rounds of assigned area and report pertinent information to Facility Director or Assisted Living Manager throughout the shift, also charts accordingly.
  9. Makes resident rounds at beginning and end of shift relaying pertinent information and fulfilling resident needs. At the end of shift, also gives verbal report to on-coming staff while making rounds.
  10. Strip and make residents' beds as often as needed.  Launder resident's linen and towels, fold and return to residents.
  11. Assist in serving meals and bussing tables during mealtimes.
  12. Assist with the activity program; conduct activities as directed by the Activity Coordinator or assigned by supervisor when needed.
  13. Document information pertaining to changes in residents' functional status, personal needs, appointments, etc.
  14. Provide one-on-one support/visitation to assigned residents on a weekly basis or as requested by supervisor or as specified in the assistance plan or daily communication log.
  15. Identify supplies needed to perform tasks and report needs to supervisor.
  16. Practice safety in working around and with equipment, chemicals, tools and utensils.  Using the buddy system with a challenging resident.
  17. Carry out other duties as assigned by supervisor.
  18. Acknowledges and assists others promptly and courteously, refers to other departments as appropriate, and demonstrates compassion allowing time for residents and significant others to verbalize.
  19. Performs other duties as assigned.
