Responsible for the coordination and supervision of Home Health Aide Services for Hospice
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Manages HHA placement through supervision of assigned support staff. Responsible for the timely placement of HHA per Hospice policy. Coordinates care priorities through communication with Team Managers and Admission Department. Responsible for tracking and/or conducting required yearly in-services for HHA employees of Hospice. Responsible for coordinating yearly competencies for HHA employed by Hospice, as well as yearly written evaluations. Verifies vendor HHA credentials prior to assigning to hospice patients. Performs periodic audits of HHA vendors’ records to assess HHA health compliance and competencies. Verifies the ability and care skills of newly hired Home Health Aides through direct observation during a home visit. Supervises the Home Health Aide within 24 hours of assignment, as needed, if the clinical office RN requires assistance. Performs all other relevant duties/special assignments as assigned. Maintains competency by participating in continuing education specific to the role and functions within the Hospice.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Licensed Registered Nurse currently registered with New York State. BSN prepared required. Experience in hospice and/or home care preferred. Effective interpersonal skills, with the ability to articulate verbally and in writing and the capacity to work with all levels of staff and management. Must have computer skills. Demonstrated teaching, public speaking and professionalism to represent hospice in community. Willingness to flex work hours as needed. Willingness to travel between all offices. Provides own transportation and possesses a current, valid New York State driver’s license.
