The Patient-Centered Medical Home (PCMH) Care Coordinator (PCMH-CC) will work on a multidisciplinary healthcare team in a primary care setting.  The PCMH-CC will focus on coaching and coordination of care for designated high-risk patients.  The PCMH-CC will work alongside the physicians and office staff in a PCMH, focusing on identifying the needs of complex patients while assisting the practice to develop a process for managing its patient population.  This person will promote patient-centered care, work side by side with primary care providers (PCPs) and medical home team members, and actively participate in multidisciplinary patient-centered team meetings.  The PCMH-CC plays a key role in transforming the medical practice into a true Patient-Centered Medical Home.

 

This position requires a working knowledge of the following:

-Principles of utilization management

-Case/care management principles

-Health care contracts and benefit eligibility requirements

-Hospital structures and payment systems

 

Skills and Abilities:

·       Basic PC skills (MS Word/Excel)

·       Data analysis skills

·       Highly developed interpersonal skills, including motivational interviewing techniques, to effectively interact and motivate change behavior

·       Provide patient and caregiver education

·       EMR experience preferred

Critical Competencies:

·       Strong reasoning, problem solving and analytical skills

·       Ability to multitask and prioritize work in a high volume environment with minimal supervision

·       Work independently as well as in a team environment

·       Function within the standards of practice for case management

·       Exercise sound judgment

·       Take initiative in finding solutions to difficult and/or sensitive problems

·       Interpret clinical reports

·       Assess patient needs and develop care plans in collaboration with the medical home PCP

·       Function in a matrix environment

·       Apply an understanding of business fundamentals and administrative expense management for day-to-day decision making

Qualifications:

·       BSN degree required. NJ RN License Required.

·       Five to eight years of direct patient care nursing experience required

·       Certified Case Management (CCM) preferred

·       Bilingual a plus

·       Reliable transportation and Valid Drivers License
