POSITION DESCRIPTION:

The Behavioral Health Care Manager Preceptor is responsible for precepting and mentoring the Behavioral Health Care Manager team and providing their orientation and training. This position identifies training needs, assists in developing training material and delivers on-going training. This includes accountability for efficient patient/workflow, customer satisfaction, staff satisfaction, productivity and achievement of operational goals as defined by program leadership.  Responsibilities also include providing behavioral health care management using the Collaborative Care Model for adult patients under minimal supervision. Develops and delivers provider and staff training and education needs with regard to Behavioral Health.  

 

 

ESSENTIAL JOB FUNCTIONS:

1.       Works with Behavioral Health leadership to continuously evaluate processes, identify problems and propose/develop process improvement strategies to enhance behavioral health care management and Patient Centered Medical Home delivery of care model.

2.       Reviews current literature regarding effective engagement and communication strategies, behavioral health interventions and behavior change strategies and incorporates into clinical practice.

3.       Works with Behavioral Health Program Manager in the assessment of new employee learning needs and development of employee orientation and education plans to ensure consistency in implementation to all new care managers.

4.       Coordinates and facilitates onboarding needs for all new care managers, evaluates orientee's progress and provides ongoing constructive feedback to both orientee and Behavioral Health Program Manager in a timely manner.

5.       Assists in the coordination, planning, delivery, evaluation, and integration of clinical education and professional development activities to foster and maintain evidenced-based behavioral health best practices.

6.       Integrates patient/program educational activities into behavioral health team meetings and continuing education programs.

7.       Develops and delivers behavioral health education modules for internal and external customers.

8.       Completes behavioral health care manager audits regularly to support Behavioral Health Program Manager evaluation and identifies behavioral health care managers that would benefit from additional mentoring.  Maintains required documentation.

9.       Collaborates with department leadership in developing policies, standards, procedures or protocols, as appropriate.

10.    Facilitates ongoing development of organization, time management and overall behavioral health care management skills of the behavioral health care managers.

11.    Evaluates and manages day-to-day workflow and adjusts to ensure increased efficiency.

12.    Provides behavioral health care management for adult patients, using the Collaborative Care Model, under minimal supervision, as described in the Behavioral Health Care Manager Job Description.

13.    Supports other offices, attends required meetings and training, and participates in committees as requested.

14.    Performs other duties as assigned and works within scope of certification/licensure.

ESSENTIAL QUALIFICATIONS:

EDUCATION:  Master's of Social Work

CREDENTIALS/LICENSURE:   LMSW in clinical or macro

MINIMUM EXPERIENCE:   3 years direct clinical patient care in behavioral health.  1 year experience teaching or training in behavioral health topics.

 

 

POSITION REQUIREMENTS (ABILITIES & SKILLS):

2.       Ability to effectively support department leadership with the behavioral health team, providing leadership, feedback and guidance to staff and demonstrating effecting conflict resolution.  Ability to affect change.

3.       Knowledge of behavioral health collaborative care program procedures and policies related to position responsibilities.

5.       Ability to work autonomously. Critical thinking skills and ability to analyze complex data sets. Ability to manage complex clinical issues utilizing excellent assessment and triage skills and protocols. Ability to implement evidence-based interventions and protocols for chronic conditions.

6.       Proficient in medical terminology.

7.       Proficient in operating a standard desktop and Windows-based computer system, including but not limited to, electronic medical records, email, e-learning, intranet, Microsoft Word and Excel,  and computer navigation.  Ability to use other software, such as NextGen Care Management Template, as required while performing the essential functions of the job.

8.       Ability to maintain required documentation.

9.       Excellent communication skills in both written and verbal forms, including listening and proper phone etiquette.

10.    Demonstrated skills in influencing and negotiating individual and group decision-making.

11.    Ability to speak before groups of people with excellent interpersonal and presentation skills.  Displays positive listening and relationship-building skills.

12.    Ability to work autonomously and collaboratively in a team-oriented environment; courteous and friendly demeanor.

16.    Ability to coordinate, plan, evaluate and integrate clinical education and professional development activities that foster and maintain care management best practices.

17.    Ability to handle patient and organizational information in a confidential manner.

18.    Knowledge of the compliance aspects of clinical care and patient privacy and best practices in medical office operations.

19.    Ability to travel to other office/practice sites and meeting and training locations.

20.    Successful completion of IHA competency-based program within introductory and training period.

 

 

MINIMUM PHYSICAL EXPECTATIONS:

1.       Physical activity that often requires keyboarding, phone work and charting.

2.       Physical activity that often requires extensive time working on a computer.

3.       Physical activity that often requires handling and lifting patients walking, standing, bending, stooping, reaching, climbing, kneeling and/or twisting.       

4.       Physical activity that may require lifting, pushing and/or pulling up to 20 pounds.

5.       Specific vision abilities required include close vision, depth perception, peripheral vision and the ability to adjust and focus. 

6.       Manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment.

7.       Must hear and speak well enough to conduct business over the telephone or face to face for long periods of time in English.

 

 

MINIMUM ENVIRONMENTAL EXPECTATIONS:

This job operates in a medical facility and requires regular walking to various locations around the hospital/clinic. Employees will be working where there is patient care equipment. Hazardous materials, including bloodborne pathogens and bodily fluids are also present. Exposure to sharps, x-rays, patients' conditions and some unpleasant sights, smells and contagious diseases is possible.  This position requires significant interaction with people (many of whom are scared, hurt and/or ill) which can be stressful and result in competing priorities.
