Reports to Practice Manager/Supervisor and works in close cooperation with and under clinical supervision of an RN or physician. Performs a wide variety of patient care activities and accommodative services to support a defined patient population program (Payer Programs, Primary Care Medical Home, NCQA accreditation) and/or service. Collaborates and coordinates program with multidisciplinary team to provide quality patient care. Ability to use various technologies, electronic medical records to include patient portal, data base reporting, Excel or Access registries and other vendor's web based technologies to collect and report various quality or care coordination based data. Demonstrates excellence in practice characterized by quality patient care and participation in formal and informal patient and staff education. Provides developmentally appropriate psychosocial and clinical care. Performs other duties as assigned.
Experience, Education and Training Minimum: Professional knowledge of nursing theory and practice. Current State license as a Licensed Practical Nurse with 3 - 5 years clinical experience in a practice setting preferred. Pediatric and program coordination experience preferred. CPR Training required and/or must be obtained within 45 days of hire. Ability to travel. Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing.
