Provides skilled nursing care to clients in a home setting or facility. Must have ability to apply nursing principles and practices. Makes the initial evaluation visits and regularly re-evaluates and assists the patient and caregiver in meeting related nursing needs. Initiates the plan of care and care plan, makes necessary revisions, furnishing those services requiring substantial and specialized nursing skill. Initiates appropriate preventative and rehabilitative nursing procedures. Initiates appropriate chart documentation and able to prepare clinical and progress notes.
Must be a graduate of an accredited LPN program and currently licensed in the State of Michigan. One year as LPN in a home health or related field and clinical nursing experience preferred. Must have effective communication skills. Will spend the majority of work time in conversation with patients and other healthcare members. Must be able to work in a self-directed and collaborative environment. Must exhibit strong organizational, documentation and teaching skills. Must be able to orientate self with agency procedures and policies, may be required to spend a significant amount of time in the car, willing to drive in all types of weather. Must be able to climb stairs, stand, sit, bend and lift.
