Meadville Medical Center is hiring for a Recovery Service Navigator job to work full-time days at Community Care Network.

The Recovery Service Navigator will work with MMC staff to develop and implement a program specific to Opioid Overdose Recovery.

Recovery Service Navigator Education and Experience Requirements:

·         Bachelor's degree in social work, social/behavioral sciences, or related field

·         Previous experience working in a support capacity within a human service based organization preferred but not mandatory

·         Knowledge and understanding of the recovery supports available in Crawford County, the locations, and how to access the services

·         Ability to make decisions and exercise sound judgment, tact and poise as a representative of Community Care Network and Meadville Medical Center

·         Able to problem-solve with clients and colleagues and reach reasonable conclusions for maximum satisfaction

·         Able to communicate effectively and maintain and preserve client confidentiality

·         Must have the written and verbal communication skills necessary to effectively interact with other members of the multidisciplinary/ healthcare team, as well as referral sources, 12-step support groups, and patients and their significant others

·         Experience working with Excel for encounter tracking and compiling reports preferred

·         A basic knowledge of local social service agencies is recommended

·         Attend core courses as outlined by Stepping Stones and CCDAEC. BLS required

Recovery Service Navigator Physical Demands and Working Conditions:

·         Receives, screens, and advises potential clients for addiction service needs

·         Engage with identified Naloxone reversed patients and those treated for possible overdose brought to the hospital emergency room

·         All contact will adhere to 42CFR part 2 and PA code 255.5

·         Conducts screening in person and via phone with potential patients, and assists in making referrals to support agencies or collateral services

·         Attends multi-disciplinary and unit meetings for the purpose of collaboration on scheduled admission, updating insurance status and providing input regarding patient care coordination

·         Accepts urgent calls and advises caller toward appropriate intervention

·         Maintains proper handling of confidential information

·         Serves as a member of a multidisciplinary team to assist in care coordination program development and delivery

·         Provides leadership for other team members

·         Provides quarterly reports of progress to CCN management, and maintains catalog of patient demographics and requested services

He/She will act as liaison between professional staff, patients, their family, and outside agencies. Effectively collaborates with all members of the health care team, including outside agencies, to develop and implement a multidisciplinary plan that delivers patient-centered care in a manner consistent with safe, efficient, and cost-effective resource utilization.
