PURPOSE: Responsible for leading the implementation, coordination and monitoring of all pharmaceutical care to the critical care areas. Participates in education and creation of competency assessments of pharmacist staff also staffing in critical care settings on critical care topics. Assists pharmacy manager in P&T related activities including new as it pertains to the critical care setting. Ensures the accurate and appropriate use of pharmaceuticals for patients of all ages in the hospital within the standards of professional practice and in accordance with the policies and procedures of the Pharmacy. Reports to the Pharmacy Supervisor and / or Pharmacy Manager. JOB DUTIES: Serve as staff pharmacist when needed including weekends, holidays and/or evenings. Assist manager in the development, review, implementation, and maintenance of Critical Care Policies and Procedures. Calculate drug doses, including pharmacokinetic dosing. Review patient drug regimens, interpret clinical data, identify drug interactions or incompatibilities, and consult with prescriber, nurse, or other health care professional as needed in the critical care setting. Conduce drug usage evaluations as they pertain to formulary items primarily utilized in the critical care setting, communicate findings to P&T or other committees as requested , and participate in process improvements based on identified opportunities for improvement in ordering and pharmacy practices in the critical care setting. Develop and participate in quality improvement efforts relating to medication management in the critical care setting. Provide education and coordination of critical care policies to other pharmacists staffing in the critical care setting. MINIMUM QUALIFICATIONS: The position requires a Bachelor of Science degree or Doctor of Pharmacy (Pharm.D) degree (preferred), The position requires one year of prior experience in hospital pharmacy, A pharmacy residency or BCCCP (Board Certified Critical Care Pharmacist) preferred, The position must have a current, valid license as a registered pharmacist in the state of Pennsylvania, The position requires familiarization with administrative and functional concepts of drug distribution and control, drug therapy practices, and clinical aspects of hospital pharmacy practice, Must be able to read and interpret printed or handwritten medication orders and be familiar with the use of computers, Must possess good communication and supervisory skills, and be able to collaborate with physicians and other healthcare workers.
