Current Licensure in the State of Ohio as a Registered Nurse with a minimum of five year's related hospital experience; BSN preferred; current driver's license required.
In an environment of continuous quality improvement, the Clinical Care Coordinator is responsible for the individualized management of hospital health care services necessary and appropriate to the provision of high quality patient care within fiscally expedient time frames.
1. Performs patient assessment to identify pertinent patient problems/needs by direct interaction with the patient and by screening/reviewing of the patient's medical records. Determines appropriateness of admission and severity of illness compared to pre-established criteria standards utilized by all third party payers.
2. Collaborates with the multidisciplinary care management team regarding the
individualization of the patient care plan in order to promote efficient use of hospital resources, facilitate timely discharges, and promote quality patient care.
3. Collects and records all clinical data, based on predetermined quality and outcome indicators developed by the medical staff, case management director and administrative director, clinical information services.
