Graduation from a recognized school of nursing. Registered, and in good standing, with the State of Michigan.
Master’s degree in clinical nursing or education required. A minimum of 6 years of nursing with 2 years of acute care, 2 years of home health and current high-level clinical expertise. OASIS Certification preferred, must obtain within 6 months of hire. Highly skilled and experienced professional RN with demonstrated ability to be responsible for an agency’s education and training needs. Proven ability to understand short and long term changes in technology, demographics, education practices and adult learning theories. Demonstrated ability to effectively communicate orally and in writing. Self-directed professional who is able to identify educational needs, develop programs, evaluate results, and provide information throughout the service area to all home health staff with minimal supervision. Knowledge of nursing theories, research and current trends. Ability to use standard office equipment, i.e. computer, fax, copier, A-V equipment. Possesses current Michigan motor vehicle license, ability to drive a car, and an insured vehicle capable of transport to patient homes or other offices in various weather conditions. Knowledge of Conditions of Participation and CHAP standards.
Possess current CPR Certification at instructor level. Meets MHH established requirements for proof of health status. Physically able to lift 35 pounds. Demonstrates ability utilize good body mechanics or lifting techniques to avoid manipulating more than 35 pounds. Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.
Develops, implements, and evaluates an agency-wide education plan related to findings from quality measurement and analysis, annual discussions, needs assessments, and performance evaluations.
Supervises clinical specialists and specialty programs.
Develops, implements, evaluates and improves specialty programs in collaboration with clinical specialists.
Implements annual education programs and competency testing as required by regulations, standards, and policies.
Works collaboratively with managers and staff to evaluate their educational and clinical needs through competency assessments.
Assures written education plans for general orientation to home health is complete and meets regulatory standards and policies.
Assures patient education materials are up-to-date and complete.
Leads and/or participates in committees, task forces, process improvement teams, and activities to assist in the development of staff.
Chairs Clinical Practice Committee.
Acts as clinical resource for field and/or office clinicians.
Coordinates annual educational programs and competency testing required by regulations, standards, and policies.
Responsible for coordinating updates to patient teaching materials.
Participates in manager-on-call rotation.
Complies with standard precautions, infection control, and safety procedures.
Employees with e-mail are required to maintain proficiency in the basic functions of the program and are also required to regularly check email and keep calendars up to date.
Performs all other duties and responsibilities as assigned.
